Reason for Visit: Referred By: Patient seen today for hearing evaluation. Questions decreased hearing bilaterally. She is seen post-cerumen management with ENT. Reports minimal perceived communication difficulty. Tinnitus and current vertigo are denied. Reports a remote history of vertigo over 10 years ago which resolved without intervention. Denies family history of hearing loss. No significant noise exposure history. Mild - moderate sensorineural hearing loss 2-8 KHz. Left:: Hearing within normal limits through 1000 Hz. Moderate - moderate/severe asymmetric sensorineural hearing loss 1500 - 8000 Hz. Fair, asymmetric word recognition. Speech Audiometry:  Right:: SRT: 25 dBHL; WRS: 100% (70 dBHL) Left:: SRT: 30 dBHL; WRS: 60% (75 dBHL) Reliability:: Good Barriers to Learning:: No Recommendations: Results reviewed and understood.
